On some of these forms you will see you have to fill in a "Club Keycolour" which is an authenticator for your club. Each club will be sent the name of a colour for it to use to authenticate its reports. The forms also ask you to give an e-mail address to which a copy of the form will be sent for your records and to act as a receipt.
There is no longer a form for registering players. Instead you use a 'play-cricket' web based system as explained in the links in the section below.

* Since there is no longer a league supplied result sheet that you have to fill in (apart from the ECB team sheet), pitch marks, etc. probably need to be written down somewhere before you leave the ground and before going on-line to send in the reports etc. I was asked to suggest a paper form. Above, under home and away game record sheets, are examples we use at Upton St Leonards. The top half of each records players, match fees and tea money etc, torn off and given to our treasurer. The bottom half is handed to whoever is dong the web result. The away sheet is printed on the back of the home sheet. I have only linked MS doc versions, because if you want something like these you really should download and adapt them for your club.
