GCCL Disciplinary Report

If the County League is to act on a disciplinary report you must have the backing of your club committee and chairman (or secretary in his absence).
You need to fill in one report for each person being reported on. If you have other witnesses, please get them also to complete a report.

Describe any continuation, or any further incidents, involving reportable behaviour by this person, stating when they occurred
 
Describe any relevant action taken by your captain before, during or after the first incident
 
Describe any relevant action taken by the opposition captain before, during or after the first incident
 
Describe any relevant action taken by umpires (including player-umpires) before, during or after the first incident
